What Happened?

On Oct. 24, 2025, HAP discovered that an unauthorized individual gained access to a business email account through a phishing scheme. An internal investigation confirmed that protected health information (PHI) was present in the compromised email account. While it is not certain that the attacker accessed all PHI, HAP notified affected members out of caution.

The breach was reported to the U.S. Department of Health and Human Services. According to official disclosures, 1,059 individuals in the United States were affected.

A notification of the breach was also posted to their website.

  • Social Security numbers
