What happened?

On June 13, 2025, Golden State Radiation Oncology learned from Integrated Oncology Network, a company that provides administrative services, that an email phishing incident that resulted in unauthorized access to patient information. The breach occurred between Dec. 13, 2024, and Dec. 16, 2024 when unauthorized parties accessed email and SharePoint accounts.

The data breach compromised both personally identifiable information (PII) and protected health information (PHI). Golden State Radiation Oncology disclosed the data breach to the U.S Department of Health and Human Services on June 27, 2025, reporting 2,130 patients impacted.
