What Happened?

On Dec. 5, 2025, Enhabit was notified by Doctor Alliance of a potential data security incident that may have impacted certain Enhabit patient information. Doctor Alliance provides a platform that transmits medical information between doctors and home health agencies or hospices.

The investigation revealed that between Oct. 31, 2025, and Nov. 6, 2025, and again between Nov. 14, 2025, and Nov. 17, 2025, an unauthorized party accessed the Doctor Alliance platform using compromised credentials for a valid user account.

Following the discovery, Doctor Alliance informed Enhabit and other providers that patient information may have been subject to unauthorized access.

The breach was reported to the U.S. Department of Health and Human Services on Feb. 5, 2026, and affected a total of 22,552 people in the United States. Enhabit has also posted a notice of the incident on its website.

Below are the exposed information types. It is important to note that neither Social Security numbers nor financial information was accessed.

  • Physician names
  • Medical record numbers
  • Certain clinical information
