What Happened?
On Aug. 8, 2025, Elevation Hospice of Utah through its parent company Three Oaks Hospice, discovered unauthorized activity in its email environment.
The company began an investigation, working to contain and remediate the issue. After an extensive review, it was determined on July 20, 2026, that unauthorized access may have affected personal information.
The breach was reported to the Texas Attorney General on Sept. 17, 2026. At least 437 people in Texas were impacted, and the company notified affected individuals by U.S. mail.
