What Happened?
Between June 5, 2026, and July 24, 2026, an unauthorized party gained access to a single clinical user account within the WellSky electronic medical record system by using compromised credentials.
The incident was discovered on July 21, 2026, leading to Doctor's Choice Home Care beginning an investigation with WellSky. The breach was limited to the WellSky platform.
The breach was disclosed to the Texas Attorney General on Sept. 21, 2026. It affected at least 14,333 people in Texas.
