The April 2025 Data Breach

The types of information exposed in this breach may include:

  • First and last name
  • Contact information
  • Age
  • Biological sex
  • Date of birth
  • Service date
  • CPT or diagnosis code
  • Medical record number
  • Name of insurance
  • Doctor’s name

For more details, you can review the official disclosure on the California Attorney General’s website.

Your Rights and Next Steps

If you received a notice from Arbor Associates or believe your information was involved, there are important steps you can take. First, review account statements and explanation of benefits forms for any errors or unfamiliar activity. If something looks suspicious, report it to the relevant financial institution or company immediately.

You are entitled to a free copy of your credit report from each of the three major credit bureaus every 12 months. Visit www.annualcreditreport.com or call 877-322-8228 to request your reports. You may also contact Equifax, Experian or TransUnion directly.

Consider placing a fraud alert on your credit file. This is free and will stay on your file for at least one year, alerting creditors to verify your identity before opening new accounts. You can also place a security freeze on your credit file, which prevents new credit from being opened without your authorization.

If you detect or suspect identity theft, report it to law enforcement, your state attorney general and the Federal Trade Commission. You can find more information about your rights under the Fair Credit Reporting Act on the FTC website.
