What Happened?
On May 22, 2025, Academic Urology & Urogynecology of Arizona discovered unauthorized access to its network. The breach was the result of a ransomware attack by a group known as INC RANSOM, which claimed to have obtained sensitive data and posted about the hack on the dark web on June 17, 2025.
After a forensic investigation, the company determined on Jan. 30, 2026, that files containing personal and protected health information may have been accessed or acquired between May 18, 2025, and May 22, 2025.

The breach affected 73,281 individuals in the United States, including residents of Maine, New Hampshire, Rhode Island, and Vermont. The company notified affected individuals in writing starting Feb. 12, 2026.
The company also posted a notice of the breach on its website. The HHS was also notified of the event.
