TriWest Healthcare Alliance, a Phoenix, Arizona-based company that administers military and Veteran health care programs for the U.S. government, disclosed a data breach that occurred in April 2026.

The breach was reported to the California and the Texas attorneys general, with 2,408 Texas residents identified as affected so far.

On April 16, 2026, TriWest Healthcare Alliance discovered unauthorized activity on its network. An unauthorized person accessed and downloaded some of the company's information.

The types of information that may have been exposed included names, Department of Defense Benefits Numbers, ZIP codes, types of authorization requests (such as physical therapy) and health insurance information.

TriWest began notifying affected consumers on July 2, 2026.

TriWest Healthcare Alliance's response to the breach

TriWest secured the services of Experian to offer 24 months of free identity monitoring through Experian IdentityWorks.

Affected individuals can enroll by visiting the Experian IdentityWorks enrollment page and entering their personal activation code before Oct. 31, 2026, at 11:59 p.m. UTC.

For questions or to report suspected fraud, affected individuals can call 1-833-918-1296 toll-free. The line is available Monday through Friday from 8 a.m. to 8 p.m. Central Time, except for major U.S. holidays.
