
Premier Medical Group of the Hudson Valley P.C., a multi-specialty medical practice headquartered in Poughkeepsie, New York, disclosed a data breach involving unauthorized access to files containing patient information.
Premier Medical Group became aware of an incident that disrupted the operations of some of its IT systems. The disruption prompted an investigation into what had occurred and whether any patient information had been affected.
The investigation determined that an unauthorized party gained access to some of the files stored on the practice's systems on June 14, 2026. After learning about this unauthorized access, the company worked to determine the full nature and scope of the information that was involved in the incident.
On July 14, 2026, the investigation reached its conclusions about what data may have been exposed.
The types of patient information potentially exposed in the breach included names, contact information, dates of birth, health insurance information, provider names, internal patient identification numbers, dates of service, medication information and treatment or diagnostic information
The company began notifying affected patients on Aug. 21, 2026, and posted a notice of the data security incident on its website. The total number of patients affected by the breach remains undisclosed at this time.
Premier Medical Group set up a dedicated incident response line for patients who have questions about the breach or need additional information.
