NSE Insurance Data Breach Exposes SSNs and Medical Records

NSE Insurance Agencies Inc., an independent insurance agency based in Exeter and Tulare, California, disclosed a data breach involving unauthorized access to its network in late 2025.

NSE Insurance Agencies discovered the breach on or about Nov. 28, 2025.

Between Nov. 6, 2025, and Nov. 29, 2025, an unauthorized party accessed the company's network and may have accessed and acquired certain files containing personal information.

On Dec. 11, 2025, a ransomware group known as Chaos posted on a dark web forum claiming it had obtained 500 GB of data from the organization.

After detecting the unauthorized access, NSE Insurance Agencies launched an investigation with the help of external cybersecurity professionals. The investigation determined on Aug. 24, 2026, that personal information in certain files may have been subject to unauthorized access and acquisition during the breach window.

The types of information potentially exposed included full names, Social Security numbers, driver's license numbers or state identification numbers, other government identification numbers, financial account information, credit or debit card information, medical information and health insurance policy information.

The breach was disclosed to the Massachusetts Office of Consumer Affairs and Business Regulation on Sept. 25, 2026. The company also posted a notice of the data breach on its website and began notifying affected consumers on Sept. 21, 2026, by U.S. Mail.

NSE Insurance Agencies is offering affected individuals a complimentary membership in Equifax Credit Watch Gold. Enrollment instructions and a unique activation code were included in the notification letters.

The line is staffed by professionals familiar with the incident and is available Monday through Friday from 9 a.m. to 9 p.m. Eastern Time, excluding major U.S. holidays.
