
Hawaii Family Dental, a locally owned dental group based in Honolulu, Hawaii, disclosed a data breach that affected approximately 45,853 individuals in the United States.
The company was founded in 1986 and operates 11 to 12 dental clinics across four Hawaiian islands, including locations on Oahu, the Big Island, Maui and Kauai. With about 300 team members, the company provides general and specialty dental care to patients of all ages.
The breach was disclosed to the U.S. Department of Health and Human Services on Aug. 21, 2026. Hawaii Family Dental also posted a notice of the incident on its website.
The company discovered the breach on July 20, 2026.
The breach resulted from an unauthorized person using a compromised account to access patient information stored on the company's systems on July 19 and 20, 2026.
On July 31, 2026, a ransomware group known as Qilin publicly claimed responsibility for the attack on the dark web. The group stated it had obtained the organization's data and intended to publish it within six to seven days.
The types of personally identifiable information that may have been accessed include names, dates of birth, phone numbers, email addresses and mailing addresses. The breach also potentially exposed protected health information, including dental insurance information and portions of medical and dental treatment information.
According to the company's notification, Social Security numbers and financial account information were not involved in the incident.
For patients who have questions or concerns, the company has set up a dedicated, toll-free assistance line. Affected individuals can call 1-888-693-3051, available Monday through Friday from 8 a.m. to 4:30 p.m. Hawaii Standard Time, or reach the company by email at privacy@hawaiifamilydental.com.
Individuals who have not yet received a notification letter but believe they may be affected are encouraged to reach out through these channels.
