In August 2025, Leidos QTC Health Commercial Services d/b/a First Rehabilitation Resources (FRR), a provider of independent medical examinations and health services, discovered suspicious activity within its email system. The company responded by shutting down the affected email system and related IT services, then migrating all users to a new, more secure email platform.

According to the disclosure submitted to the Massachusetts Attorney General on Dec. 30, 2025, the incident exposed sensitive personal and health information including names, dates of birth, Social Security numbers, driver’s license numbers, government ID numbers, and medical or health insurance information.

This breach is considered severe due to the type of data involved. Both personally identifiable information (PII) and protected health information (PHI) were potentially accessed. The breach was contained after the company worked with cybersecurity experts to ensure the unauthorized party no longer had access to its systems.

Leidos QTC Health Commercial Services’ response

For those affected, FRR is offering 24 months of complimentary credit monitoring and identity protection services through Epiq – Privacy Solutions ID. The package includes three-bureau credit monitoring, VantageScore credit scores, Social Security number monitoring, dark web surveillance, change of address monitoring, credit freeze assistance, personal information protection, identity restoration, lost wallet assistance, and up to $1 million in identity theft insurance.

Individuals are encouraged to:

  • Review account statements, credit reports, and explanation of benefits for unauthorized activity
  • Place a fraud alert or security freeze on credit files if suspicious activity is detected
  • Take advantage of the free credit monitoring service provided

The company has provided detailed instructions and contact information for enrolling in these services and for obtaining additional support.
