Decisely Insurance Services has reported a major data breach. The cybersecurity incident was discovered on December 17, 2024 and an investigation revealed that data may have been acquired by an unauthorized actor on December 16, 2024.
The data breach involved unauthorized access to personally identifiable information (PII) and protected health information (PHI). Exposed information may include names, dates of birth, phone number, passport number, digital signature, and Social Security numbers. According to the disclosure filed with the U.S. Department of Health and Human Services, 65,405 individuals were affected.
Decisely Insurance Services published a Notice of Data Incident on its own website and notified affected individuals by mail on June 13, 2025. Impacted consumers include 682 Massachusetts residents, 4,915 Texans and 75 New Hampshire residents.
The data breach was disclosed to the California, Massachusetts, Texas, New Hampshire and Vermont Attorney Generals' offices beginning June 14, 2025.
