







Decisely Insurance Services has reported a major data breach. The cybersecurity incident was discovered on December 17, 2024 and an investigation revealed that data may have been acquired by an unauthorized actor on December 16, 2024. The cybersecurity incident impacted a total of 113,984 individuals.
The cyberattack compromised sensitive customer information, including names, phone numbers, dates of birth, Social Security numbers, passport numbers and digital signatures. This combination of exposed data puts individuals at risk for identity theft and fraud.
Decisely disclosed the data cybersecurity incident to the U.S. Department of Health and Human Services on June 13, 2025 and began notifying affected individuals by mail. The insurance company also published a Notice of Data Security Incident on its website.
The data breach was disclosed to various state Attorney Generals' offices between June 14, 2025 and Oct. 8, 2025.
