Decisely Data Breach Exposes Sensitive PII Details

Decisely Insurance Services has reported a major data breach. The cybersecurity incident was discovered on December 17, 2024 and an investigation revealed that data may have been acquired by an unauthorized actor on December 16, 2024.

The data breach involved unauthorized access to personally identifiable information (PII). Exposed information may include names, dates of birth, phone number, passport number, digital signature, and Social Security numbers.

Decisely Insurance Services published a Notice of Data Incident on its own website and notified affected individuals by mail on June 13, 2025.

The data breach was disclosed to the California Attorney General's office on June 14, 2025 and the Vermont Attorney General's office on June 16, 2025.
