Decisely Insurance Services has reported a major data breach. The cybersecurity incident was discovered on December 17, 2024 and an investigation revealed that data may have been acquired by an unauthorized actor on December 16, 2024.

According to the disclosure filed with the U.S. Department of Health and Human Services, 65,405 individuals were affected.

Decisely Insurance Services published a Notice of Data Incident on its own website and notified affected individuals by mail on June 13, 2025.

Information Exposed:

  • Names
  • Dates of birth
  • Phone numbers
  • Passport numbers
  • Digital signatures
  • Social Security numbers

Impacted consumers include 682 Massachusetts residents, 4,915 Texans and 75 New Hampshire residents.

The data breach was disclosed to various state Attorney Generals' offices beginning June 14, 2025.

Decisely Insurance Services' response

Decisely Insurance Services launched an investigation and notified affected individuals.

  • Sign up for the free credit monitoring and identity theft recovery services through Kroll, if offered.
  • Monitor your credit reports and financial accounts for any unusual activity.
  • Be alert for phishing emails or phone calls that may use your exposed information.
  • Consider placing a fraud alert or credit freeze with major credit bureaus.

Decisely has set up a call center to answer questions about the cybersecurity incident at 866-461-3640, from 8:00 am to 5:30 pm Central Time.
