
Clement Manor, a senior living and healthcare organization in Wisconsin experienced a data breach affecting both personally identifiable information (PII) and protected health information (PHI). An investigation determined unauthorized access to the Clement Manor systems took place on or around April 14, 2025.
Exposed information included full names, addresses, dates of birth, driver's license numbers, Social Security numbers, diagnosis information, medication information, health insurance information, provider name other treatment information and financial account information.
The data breach was disclosed to the U.S. Department of Health and Human Services, the Oregon Attorney General, the Vermont Attorney General, and Clement Manor published a notice of data incident on its own website.
The number of affected individuals was determined to be 16,046, including at least 10 individuals in Massachusetts and three in New Hampshire.
In response to the breach, Clement Manor took steps to secure its systems, notified law enforcement and launched an investigation. In addition to required state and federal disclosures, Clement Manor is offering affect individuals free credit monitoring and identity protection services.
