Azle Smiles, a general and cosmetic dental practice in Azle, Texas, was affected by a ransomware attack in May 2026 that targeted Soniva Dental Care, the organization that operates the practice. The Soniva Dental Care data breach affected at least 30,000 Texans across its network of dental practices, and Azle Smiles was among the locations involved.

On May 26, 2026, the practice's IT support company informed Azle Smiles that it had identified irregularities related to remote access session hosts. The IT support company's detection of unusual activity in the remote access session hosts was the first sign that the practice's systems had been compromised.

The practice then worked with its IT support company to investigate the full scope of the incident and determine which patients' information may have been affected.

Through that investigation, Azle Smiles determined that certain personal information of patients associated with the practice may have been accessed during the breach. The incident did not affect payment information or the dental care the practice had provided to its patients.

The types of personally identifiable information exposed included names, addresses, dates of birth, driver's license numbers and government-issued ID numbers such as passports or state ID cards. The breach also involved protected health information, including medical information and health insurance information.

The breach was reported to the Texas Attorney General, with 9,444 Texas residents identified as affected. The practice posted a notice on its website.

Azle Smiles' response to the breach

The practice is offering identity protection monitoring services at no cost to affected individuals. This service is designed to help patients detect potential misuse of their personal information following the breach.
