
Agave Dental Floresville, a general and cosmetic dental practice located at 2006 10th St. in Floresville, Texas, disclosed a data breach connected to a larger data breach affecting Soniva Dental Care, the parent dental organization that operates the practice.
On May 26, 2026, Agave Dental's IT support company informed the practice that it had identified irregularities related to remote access session hosts. Within minutes, it became clear that an attack had occurred, and the IT team's primary focus shifted to containing the situation and protecting patient data.
The IT team worked to lock down external access to the remote desktop web services infrastructure and ensured that any external connections were terminated. At the same time, all user accounts with access to remote desktop web access were disabled across the practice. An office-wide password reset was also carried out to help prevent further unauthorized access to the practice's systems.
The patient record platform was restored after the incident, and archive data was not affected by the attack. However, through its investigation, the practice determined that certain personal information of patients associated with the practice might have been accessed during the incident.
The types of personally identifiable information (PII) that may have been exposed included names, addresses, dates of birth, driver's license numbers and government-issued ID numbers such as passports or state ID cards. Protected health information (PHI) that may have been exposed included medical information and health insurance information.
The company stated that payment information and dental care was not affected by the incident.
The breach was reported to the Texas Attorney General, with 11,790 Texas residents identified as affected by the Agave Dental Floresville breach.
The practice posted a notice on its website informing patients of the incident and the steps they can take to protect themselves.
