
AdaptHealth LLC, a home healthcare equipment and medical supply company based in Conshohocken, Pennsylvania, disclosed a data breach that occurred in June 2026.
AdaptHealth first became aware of the breach on June 15, 2026, that an unauthorized third party had gained access to certain company systems. The investigation found that the attacker first gained access on June 5, 2026, through a social engineering attack that targeted a single company user account.
The unauthorized party had access to the systems for approximately 10 days before the company detected the intrusion.
Upon discovery, AdaptHealth took steps to secure its systems and engaged third-party forensics experts to determine the scope of the incident. The investigation confirmed that the unauthorized party took certain data from the company's systems, including patient information.
The types of personally identifiable information exposed included names, contact information, demographic information and dates of birth. The breach also involved protected health information, including health insurance information (such as health plan names and policy numbers), health information (such as durable medical equipment order details and referring health care provider information) and passwords associated with insurance billing.
AdaptHealth revealed it does not store Social Security numbers, individual financial account information or payment card information in the affected systems.
The breach has so far affected 143,454 Texas residents and 48,090 Vermont residents. The breach was additionally reported to attorney general in California, as well as to the The company Securities and Exchange Commission.
AdaptHealth has arranged for affected individuals to receive identity monitoring services through Kroll at no cost. These services include single bureau credit monitoring, fraud consultation and identity theft restoration.
Affected individuals received an activation code and verification ID in their notification letter and can enroll at Kroll's enrollment website.
