This settlement benefits individuals whose personal identification information was compromised in a data breach that occurred between April 2, 2023, and April 17, 2023.

What Can Class Members Claim?

  • Extraordinary Losses: Two methods of third-party documentation supporting the claimed losses, along with a written description and an attestation of authenticity.

How to Claim Your Award

To claim your award, you must complete the online Claim Form available at file a claim online and submit it to the Claims Administrator no later than August 8, 2024. You can also mail the completed Claim Form to the Claims Administrator at the provided address. If you lack internet access, you may contact Plaintiff’s counsel for assistance in processing your form.

$348,000 Settlement Fund Breakdown

The total settlement amount of $348,000 will cover the cash benefits, credit monitoring services, and other related costs. The Class Representative, Raymond Gomez, will receive an enhancement of up to $3,000 for his services. The attorneys for the class will be paid up to $174,000 for their fees and reimbursement of costs.
