On April 7, 2026, LHC Group discovered that an employee may have been the victim of a vishing (voice phishing) attack.
Shortly after, a third-party technology vendor that supports LHC Group’s referral management and care coordination systems reported suspicious activity involving an LHC user account. LHC Group launched an investigation, secured its systems, worked with forensic experts, and notified the FBI.
The investigation revealed that a threat actor stole credentials and accessed a large volume of files containing patient information between April 7, 2026, and April 15, 2026.
The affected files included protected health information (PHI) and other sensitive data.
Information Exposed:
- Names of individuals
- Addresses
- Social Security numbers
- Medical information
- Health insurance information
- Dates of birth
- Full names, addresses, dates of birth, and demographics
LHC Group began confirming the identities of impacted individuals on July 9, 2026.
The breach affected at least 16,885 people in Texas alone. The company notified affected individuals by U.S. Mail.
