
Unlimited Technology Systems LLC, a healthcare software and revenue cycle management company based in Montgomery, Ohio, disclosed a data breach that occurred in October 2025. Because it is a software vendor that processes data on behalf of healthcare providers, patients of those providers may be affected even though they may have no direct relationship with the company.
The breach was disclosed to the Iowa Attorney General on July 1, 2026. Unlimited Technology Systems discovered the breach on Oct. 19, 2025. The total number of individuals affected nationwide has not been publicly disclosed.
Unlimited Technology Systems discovered unauthorized activity within its commercial datacenter, according to the notification sent to affected individuals. An investigation determined that an unauthorized actor may have obtained copies of personal information between Oct. 5 and Oct. 10, 2025.
The exposed information included both personally identifiable information and protected health information. Personally identifiable information that may have been accessed included names, Social Security numbers, dates of birth, email addresses, physical addresses, scanned documents such as copies of driver's licenses or other government identification, insurance cards and intake formsphone numbers, and other demographic information.
Protected health information involved in the breach included health insurance and patient balance information such as insurance policy numbers or claims and benefits information, and medical information such as medical record numbers, dates of service and diagnosis information.
The company states the breach did not involve full patient medical records, medical imaging or financial information such as credit card or bank account numbers.
Unlimited Technology Systems is offering two years of free identity monitoring services through Kroll, a global leader in risk mitigation and response.
To enroll, affected individuals should visit Kroll's website and enter the activation code and verification ID included in their notification letter. Enrollment must be completed before the activation deadline printed in the letter.
