Southern Illinois Ob-Gyn Associates, S.C., a women's health practice with offices in Carbondale and Herrin, Illinois, disclosed a data breach that affected approximately 38,700 individuals in the United States.

The practice's computer systems were potentially compromised by an unauthorized actor. A forensic investigation into the incident concluded on or about Jan. 28, 2026. It determined that an unauthorized party had gained access to certain personal information stored within the practice's network environment.

Following this finding, the practice began a comprehensive review of the affected data to identify which specific individuals were impacted and what types of their information may have been compromised. This review was finalized on April 28, 2026, at which point the practice completed its list of individuals to notify.

The breach was reported to the Massachusetts Office of Consumer Affairs and Business Regulation on June 5, 2026. The practice began notifying affected individuals by mail.

The types of information that may have been exposed both personally identifiable information and protected health information including Social Security numbers, driver's licenses and medical records were potentially compromised.

Southern Illinois Ob-Gyn Associates' response to the breach

Southern Illinois Ob-Gyn Associates is offering affected individuals 24 months of complimentary credit monitoring services through Cyberscout. The services include single-bureau credit monitoring, a single-bureau credit report and a single-bureau credit score.

To activate the free credit monitoring, affected individuals must visit the enrollment website listed in their notification letter and enter the unique activation code provided. Enrollment must be completed within 90 days of the date of the letter.

The practice has also established a toll-free call center for individuals with questions or concerns. The call center is available Monday through Friday from 8 a.m. to 8 p.m. Eastern time, excluding U.S. national holidays. The toll-free number is included in each individual's notification letter.

Notification letters are being sent by first-class mail from Southern Illinois Ob-Gyn Associates, care of Cyberscout, at P.O. Box 1286, Dearborn, MI 48120-9998.
