Sedgebrook, a senior living and skilled nursing company based in Illinois, experienced a cyberattack. On May 5, 2025, the company became aware of suspicious activity within its internal network. An investigation revealed that an unauthorized actor accessed the the Sedgebrook network from May 4, 2025 to May 5, 2025 and may have obtained sensitive files.

A review was completed on Aug. 26, 2025 and determined that the compromised files contacted both personally identifiable information (PII) and protected health information (PHI). Exposed information included names, addresses, dates of birth, Social Security numbers, driver’s license numbers, financial account numbers, medical treatment information, medical record numbers and health insurance policy information.

Sedgebrook began notifying affected individuals by mail on Oct. 24, 2025 and published a Notice of Data Incident on its website.

The data breach was also disclosed to the Massachusetts Attorney General's office on Oct. 23, 2025.

Sedgebrook's response

Sedgebrook responded to the incident by securing its systems, notifying law enforcement and engaging cybersecurity experts. The company is offering free identity theft protection services to individuals whose Social Security number and/or driver's license numbers were exposed in the breach.

  • Carefully review any notice or communication you receive and sign up for the free identity theft protection services, offered by the company.
  • Monitor your credit reports and financial accounts for any unusual activity.
  • Be alert for phishing emails or phone calls that may use your exposed information.
  • Consider placing a fraud alert or credit freeze with major credit bureaus.
