Nephrology Associates Data Breach Affects 24k Patients

Nephrology Associates, M.D., P.A., a kidney-care medical practice specializing in nephrology and hypertension, disclosed a data breach that affected 24,088 individuals in the United States.

The practice, which provides services including chronic kidney disease care, dialysis and hypertension management, reported that unauthorized access to its computer network occurred earlier this year.

The breach was disclosed to the U.S. Department of Health and Human Services on July 29, 2026. The company also posted a notice on its website describing the incident and the steps individuals can take to protect themselves.

The practice discovered that unauthorized access to its network occurred between Jan. 17, 2026, and April 9, 2026. During this nearly three-month window, an unauthorized party accessed the practice's computer systems and removed personal information from the network.

A comprehensive investigation and document review concluded on July 1, 2026. The review confirmed that personal data had been taken from the network in connection with the incident.

The types of information exposed included full names, Social Security numbers, dates of birth, driver's license numbers or state identification numbers, other government identification numbers, treatment or diagnosis information, and health insurance policy information.

Nephrology Associates began notifying consumers on or about July 30, 2026.

Nephrology Associates is offering complimentary credit monitoring services. Affected individuals can find enrollment details and instructions in the notification letters they received from the company.

The company urged affected individuals to remain vigilant in reviewing their financial account statements and credit reports for any signs of fraudulent or irregular activity.
