Immediate actions were taken to secure the network, and an investigation was launched with the assistance of third-party forensic specialists. It was determined that an unauthorized threat actor might have accessed certain systems as early as June 20, 2023. This breach potentially led to unauthorized access or acquisition of files containing sensitive personal data.

The compromised systems held a variety of personal information provided by the Covered Entity. This included:

• names and addresses
• medical billing and insurance information
• specific medical details such as diagnoses, medications, treatments
• demographic data like dates of birth, Social Security numbers, and financial information

M&D Capital Premier Billing LLC has taken significant steps to enhance security and prevent future incidents.
