Life Bridges Data Breach Exposes Personal and Medical Information

Life Bridges Inc., a nonprofit human services organization based in Cleveland, Tennessee, disclosed a data breach after an unauthorized actor accessed its computer systems and copied sensitive personal and medical information.

From June 17, 2026, through June 22, 2026, an unauthorized actor accessed Life Bridges' systems and copied certain data. The organization detected the unauthorized activity on the final day of the intrusion and launched an investigation.

On June 25, 2026, a ransomware group known as INC Ransom posted a claim on the dark web, claiming to have obtained data belonging to Life Bridges.

The investigation found that the breach exposed a broad range of both personally identifiable information and protected health information.

The personally identifiable information exposed included names, addresses, dates of birth, Social Security numbers, driver license numbers, financial account information and debit card information.

The protected health information exposed included diagnoses, medical conditions, lab results, treatment information, dates of service, insurance claims information, medical record numbers, Medicare numbers, Medicaid numbers and managed care organization numbers.

Life Bridges reported the breach to the Massachusetts Office of Consumer Affairs and Business Regulation on Aug. 20, 2026, as well as posted a notice on its website. The organization began sending notification letters to affected individuals on Aug. 20, 2026.

Life Bridges's response to the breach

Life Bridges is offering affected individuals complimentary credit monitoring and identity theft protection services through Experian IdentityWorks.

Each affected person received a unique activation code and enrollment deadline in their notification letter. Individuals must enroll by the deadline specified in their letter to activate the credit monitoring service.
