Horizon Eye Care Data Breach: PHI Potentially Exposed

Horizon Eye Care Laser & Eye Surgery Center, a New Jersey ophthalmology practice and ambulatory eye surgery center, disclosed a data breach that affected individuals in the United States.

The breach was disclosed to the U.S. Department of Health and Human Services on Aug. 7, 2026. Horizon Eye Care posted a notice of the incident on its website.

On or about June 8, 2026, Horizon Eye Care identified suspicious activity within certain of its computer systems. The company stated that it took action to address and isolate the issue.

Horizon Eye Care then launched an investigation to determine the nature and scope of the event.

The company's investigation has not yet been able to determine the specific individuals and information that may have been affected. Horizon Eye Care said it chose to share information about the event in the interim so that individuals may take steps to protect their information if they feel it is appropriate to do so.

The types of information that may have been exposed vary by individual but could include some combination of names, demographic information, treatment information and health insurance information. The affected systems stored data related to both current and former employees as well as patients of the practice.

Horizon Eye Care's response to the breach

Horizon Eye Care encouraged individuals to remain vigilant against incidents of identity theft and fraud. The company specifically recommended that individuals regularly review their account statements and explanations of benefits, and monitor their free credit reports for suspicious activity and to detect errors.

The company's notification included guidance on placing fraud alerts and credit freezes with the three major credit reporting bureaus: Equifax, Experian and TransUnion.

The company set up a dedicated phone line for individuals who may have questions about the incident. People can call 1-800-405-6108 between 8 a.m. and 8 p.m. Eastern time, Monday through Friday, excluding major U.S. holidays.
