Freedom Plaza Senior Living experienced a major data breach. An investigation into the cybersecurity incident revealed that email accounts were subject to unauthorized access from Sept. 6, 2024 to Oct. 3, 2024, and both personally identifiable information (PII) and protected health information (PHI) may have been compromised.

Exposed information may include first and last names, dates of birth, Social Security numbers, health insurance information, medical records and financial information. Medical records and Social Security numbers are highly sensitive and can be used for identity theft or other malicious purposes.

The data breach was disclosed to the Massachusetts and Vermont Attorney Generals' offices beginning on July 12, 2025. The total number of impacted individuals has not been released.

Freedom Plaza Senior Living is one of multiple Life Care Services (LCS) senior facilities impacted by 2024 cybersecurity incidents. Other LCS communities involved in data breaches include Regency Oaks and Freedom Square of Seminole.

Freedom Plaza Senior Living's response

In addition to required state and federal disclosures, Freedom Plaza began notifying affected individuals on July 11, 2025. Freedom Plaza is also offering free IDX identity protection services to impacted residents and staff.

  • Monitor your credit reports and financial accounts for any unusual activity.
  • Be alert for phishing emails or phone calls that may use your exposed information.
