Complete Care Rehab LLC Data Breach Affects 4,764

Complete Care Rehab experienced a data breach that affected thousands of individuals. On or around May 11, 2025, Complete Care discovered suspicious activity within its network. The data breach included both personally identifiable information (PII) and protected health information (PHI).

Compromised information may include name, address, email address, phone number, date of birth, Social Security number, medical diagnosis and treatment information, health insurance information, date of service and cost of service. At least 4,764 individuals have been affected.

Complete Care Rehab published a Notice of Data Privacy Event on its website on July 2, 2025. The data breach was disclosed to the U.S. Department of Health and Human Services on July 8, 2025.

In addition to required federal and state disclosures, Complete Care Rehab will notify affected individuals. Complete Care has also set up a dedicated response line for affected patients at 586-675-1178, between the hours of 10:00 a.m. to 4:00 p.m. Eastern time, Monday through Friday.
