CenterWell, a Humana-owned healthcare services company specializing in senior-focused care, disclosed a data breach that affected approximately 9,651 individuals in the United States, including 4,618 Texas residents.

The breach was disclosed to the attorneys general of Massachusetts and Texas, as well as the U.S. Department of Health and Human Services.

The breach resulted in the exposure both personally identifiable information and protected health information including names, addresses, dates of birth, medical information and credit or debit card numbers.
