
On Jan. 20, 2026, Alexes Hazen MD, PLLC discovered unauthorized access to its computer systems, affecting the personal and medical information of individuals in the United States. The breach occurred between June 23, 2025, and July 15, 2025, when an unauthorized actor gained access to certain computers used by the practice.
The full extent of the breach is still under investigation, but it is known that the attacker may have accessed or taken sensitive data during this period.
The information exposed in this incident varies by individual. It may include personally identifiable information (PII) such as name, demographic information, date of birth, Social Security number and government-issued identification.
In addition, protected health information (PHI) was also at risk, including medical history, condition, procedure or diagnosis information, medication details, photographs, insurance information and payment information.
The breach was reported to the U.S. Department of Health and Human Services on Feb. 12, 2026. The practice has also posted a notice of the incident on its website.
To help those affected, the practice has provided a toll-free hotline at 877-206-5545, available Monday through Friday from 9 a.m. to 5 p.m. EST.
They encourage individuals to remain vigilant by reviewing account statements, monitoring free credit reports and explanation of benefits for suspicious activity and reporting any anomalies to relevant parties such as banks or financial institutions.
