On Jan. 20, 2026, Alexes Hazen MD, PLLC discovered unauthorized access to its computer systems, affecting the personal and medical information of individuals in the United States. The breach occurred between June 23, 2025, and July 15, 2025, when an unauthorized actor gained access to certain computers used by the practice.

To help those affected, the practice has provided a toll-free hotline at 877-206-5545, available Monday through Friday from 9 a.m. to 5 p.m. EST.

They encourage individuals to remain vigilant by reviewing account statements, monitoring free credit reports and explanation of benefits for suspicious activity and reporting any anomalies to relevant parties such as banks or financial institutions.

The practice recommends taking the following steps:

  • Request a free credit report annually from each of the three major credit bureaus (Equifax, Experian and TransUnion) at www.annualcreditreport.com or by calling 877-322-8228
  • Consider placing a fraud alert or credit freeze on credit files to help prevent new accounts from being opened without consent
  • Report any suspected identity theft to law enforcement, the Federal Trade Commission or the state attorney general
